Two school nurses are assigned to serve the entire school district. In
addition to handling various health, accident or emergency
situations, school nurses are responsible for carrying out health
related programs required by the state of Kansas.
Part of this responsibility includes monitoring the documentation of
required entry information. This includes the following documents:

Current immunization records

State certified birth certificate

A health physical for every pupil up to the age of nine years that
has not previously enrolled in a Kansas school. This must be completed
within 12 months of school entry.
This information is necessary at the time you enroll your child in
school. Please be prepared to present this information when you come
for enrollment.
